How to complete the survey

This survey has been prepared solely for the use of OPSEU members in the Hospital Professionals Division.

With the assistance of the OPSEU representative(s) on the Joint Health and Safety Committee, the Local President or Highest Ranking Officer should complete the survey and return it to Lisa McCaskell, Senior Health and Safety Officer at OPSEU by November 8, 2004. You can complete the survey online and email it to lmccaskell@opseu.org or print it and fax to Lisa McCaskell at OPSEU: 416-443-0553.

The survey and letter of explanation were mailed to Presidents or Highest Ranking Officers in mid-October. Please only complete one copy of the survey per workplace. Thank you for your assistance with this important project.
